
Dementia describes a collection of symptoms that are caused by disorders affecting the brain. It is not one specific disease. Dementia affects thinking, behaviour and the ability to perform everyday tasks. Alzheimer’s disease is the most common form of dementia accounting for between 50% and 70% of all dementias.
Research tells us, the keys to living well with dementia are to remain physically active, mentally stimulated and socially connected in ways that suit a person’s abilities and interests.
People who suffer from dementia have unique and specific needs and their care and support services need to be individually tailored. At CCG we offer services that are unique and specific services to suit individual needs and allow for choice.
We can provide assistance and support with:
Help at home services: Our Help at Home Services can provide you with practical assistance including help with cleaning, shopping, transport and personal care.
Health and wellbeing services: Our Health and Wellbeing Services can assist with Accessing and designing a personal exercise program.
Dementia Advisory Service: If you have a diagnosis of memory loss or dementia and concerned about it, we can support you with advice and practical tips from experienced dementia professionals who can connect you with services, obtain a diagnosis, promote wellbeing and provide strategies to manage changes, support independence and plan for the future.
Carer and family support: Providing carer with much needed break from their caring role and focus on their own health and wellbeing and participate in other things.
Social interaction and activities: can provide you with support to remain socially active and a part of your community, reconnecting with past hobbies or interests, or joining community recreational groups.
We would happily recommend CCG and were very satisfied with the care provided for our mum. The carers were able to adapt to Mum’s changing and increased needs over time. They regularly communicated to check on how things were going & kept us involved as a family, in planning Mum’s care.
